Grant Application Process
The Board of Directors of Dubois County Community Foundation, Inc. has developed a procedure for submission and evaluation of grant proposals. The Grants Committee of the Foundation carefully reviews and considers each proposal submitted and makes funding recommendations to the Board of Directors. Evaluation is based on the project's feasibility, soundness of its implementation plan, viability of subsequent long-term financing, and fulfillment of community need.
APPLICATION PROCESS:
1. Obtain grant application form from the Dubois County Community Foundation offices.
2. Complete the "Grant Application Form," "Grant Agreement," and the "Grant Application Certification."
3. Attach additional required documentation.
4. Submit final proposal. Applications not meeting the requirements as outlined below will not be considered for funding.
5. Grants committee reviews proposal, makes an on-site visit if deemed beneficial to the review process, and makes its recommendations for funding to the Board of Directors. The Board of Directors determines the funding recipients.
6. Notification of the Board's decision will be made approximately 3-4 months after submission deadline.

Program Narrative - expansion upon the summary including program procedures, indication of whom the program will serve and an estimated number of individuals so served, personnel involved, anticipated results, and internal evaluation procedures. b. IRS 501(c)(3) Determination Letter. c. Governing Body Information - names of the organization's officers, directors, and key project personnel. d. Detailed Project Budget - identification of all projected expenses AND anticipated income associated with the program in justification of the amount being requested. e. Most recent Audit Report and/or Financial Report. f. Visual materials may be submitted as appendices. |
